This form is to be completed by Associations on behalf of players seeking exemption from playing at a 2026 State Championships/Tournament or a state trials at which they would have been considered for State Team selection.

Applications for exemption must set out full reasons as to why the player is not available for the relevant selection event. Players in the Hockey Australia National Squad, who are unable to compete, are not required to complete an exemption request form.

Exemption Requests can be submitted up until 2 days prior to respective championships or trials commencement. For requests in extenuating circumstances submitted later than this, please notify Hockey Queensland.

Hockey Queensland will review all submissions prior to providing approved requests for exemption to relevant selection panel for consideration.

If seeking exemption on medical/injury grounds, you should provide a medical certificate to Hockey Queensland, prior to the commencement of the particular State Championships or Trials event.
