I understand that Exemption Requests can be submitted up until 2 days prior to the relevant championships or trials commencement.

I understand that applications for Exemption from a Hockey Queensland Tournament will not be accepted if ALL information is not completed

I acknowledge that Applications for Exemptions submitted after the closing date will not be accepted. Supporting evidence should be included with the submission, and additional information may be requested.

All information provided in this Application for Exemption is true and accurate and I understand that approval for Exemption is at the discretion of Hockey Queensland.
