Honours Nominations
Thank you for your interest in submitting a nomination to recognise a hardworking and high achieving volunteer from your Association. The HQ Life Membership is the highest HQ honour, and recognises outstanding long term contribution towards hockey across Queensland. The Distinguished Service Award recognises substantial contribution towards one or more hockey Associations.
All nominations must be signed and submitted by a member of the Association’s Executive and will be considered by the HQ Board. Life Memberships must be passed by the Board and confirmed by a majority of members at the AGM.
Life Membership
Life Membership is awarded in recognition of those who have made an outstanding contribution and in doing so have made an extremely positive impact on the sport and/or Hockey Queensland. An outstanding contribution to Hockey Queensland is one which:
- Occurs over a significant period of time (at least 10 years)
- Results in the Organisation and sport being substantially, demonstrably, and positively changed by that contribution
- Required substantial and commendable personal input
- Contributes positively to the reputation and standing of Hockey Queensland and the sport
- An Ambassador or role model
Nominations
- Nominations will be called annually prior to the Annual General Meeting and announced at that meeting.
- Nominations will be received by Hockey Queensland at any time during the year.
- Nominations will be provided to the appropriate panel for approval and ratified at the Annual General Meeting.

Distinguished Service
Hockey Queensland’s Distinguished Service Award is awarded in recognition, by Hockey Queensland, to those who have been of service to an Association or to the sport of hockey, through one or more Affiliated Associations, over periods totalling at least fifteen (15) years.
- Occurs over a significant period of time (at least 15 years)
- Results in positive change in one or more Associations greatly benefitting the hockey community
- Required substantial and commendable personal input
- An Ambassador or role model
Nominations
- Nominations will be called annually prior to the Annual General Meeting and announced at that meeting.
- Nominations will be received by Hockey Queensland at any time during the year.
- Nominations will be provided to the appropriate panel for approval and ratified at the Annual General Meeting.
