Assumption of Risk and Liability Waiver

I, the undersigned parent/guardian of the above-named child, acknowledge that participation in the Hockey Tots program involves physical activity and carries some risk of injury. I understand that all reasonable care will be taken by the organisers, coaches, and volunteers to provide a safe environment.

By registering my child for the Hockey Tots program, I:

• Voluntarily allow my child to participate in all program activities.
• Accept responsibility for any risks associated with participation, including minor injuries that may occur through normal physical activity.
• Confirm that my child is medically fit to participate and that I will notify the organisers of any relevant medical conditions, allergies, or special requirements.
• Release and hold harmless the organisers, coaches, volunteers, and venue providers from any liability for injury, loss, or damage arising from my child's participation, except where caused by negligence.

Medical Treatment Consent

In the event of an accident or medical emergency, I authorise HQ program staff to seek appropriate medical treatment for my child if I cannot be contacted immediately. I understand that any associated medical expenses are my responsibility.
