This form is to be completed if you wish to nominate for selection in the HQld Country team to attend the 2022 HA Country Championships in Albany, WA, to be held from 6 to 13 August, 2022. The selection process will be undergone at the HQC. The nomination fee is $33. This is a non refundable nomination fee.

Update 29/03/22: Please note the updated eligibility requirements for players as directed from Hockey Australia:

• I reside in a country region of Queensland
  • exemption: A player living in Brisbane for the purpose of study having moved here from a country region is exempt from this rule
• AND I play in a country region of Queensland
• AND I do not play in Division 1 of BWHA or BHA competition 
  • up to three players can be exempt from this rule as long as they fulfil the remaining requirements
• AND I have not represented QLD or any other state/territory at National U21 Championships or Hockey One in the previous year
  (Note – any player who played U21/Hockey One for another state/territory/franchise through HA’s Import Process is eligible for selection)
