This form is to be completed to nominate for selection in a HQ Women’s Masters team to attend the 2022 Hockey Australia (HA) Women’s Masters Championships in Cairns to be held from Thursday, 22^nd September to Saturday, 1^st October, 2022. Nominations close on Friday 3rd June.

Player Commitment

• Players selected in a Qld team, must assemble in Cairns by 6.00pm on Sunday, 18th September.

• The player levy is expected to be $1800 (plus air fare and uniforms).

NB All players are required to purchase new uniforms in 2022. Previous HQ representative uniforms including BLK playing uniforms, polos and track suits cannot be worn at the Championship.
Sample sizes for HQ uniforms will be available at the State Championships on Thursday 9^th June 11.00am-3.00pm and Friday 10^th June 9.00am and 1pm. All nominees are encouraged to try sample uniforms.

• The teams and shadow players will be named on HQ website on Wednesday 15th June.
• Players will be emailed their selection letter, including a confirmation link, shirt number and uniform ordering details and first invoice by Friday 17th June.
• All team players must confirm their position in their team by Friday 24^th June. Players who have not confirmed by 24^th June may be replaced.
• Shadow Players will be emailed their selection letter including a confirmation link by Friday 17^th June. Shadow Players will not receive an invoice or uniform ordering details unless called into a team. Shadow players must confirm their position by Friday 24^th June.
• To confirm their position in a team, players must, by 24^th June pay the first deposit invoice of $900. A full breakdown of expenses will be listed in the second/final invoice which will be sent by 5^th August and must be paid by 19^th August.

Any player seeking permission to be on a payment plan must confirm their position and pay the first deposit invoice of $900 by Friday 24^th June and contact HQ prior to Friday 24^th June to establish the plan. All scheduled payments for players on a plan must be up to date on Friday 19^th August.

Accommodation has been booked for Saturday 1^st October. Players may leave the Contingent following the Closing Ceremony but refunds are not given for unused accommodation.

The current Hockey Australia Vaccination Policy requires all players to provide proof of vaccination before attending the Australian Championships.

Nomination Information

• All nominees must have paid the 2022 HQ Senior Player Levy.
• The $40.00 nomination fee is non-refundable.
• Late nominations and nominations which do not include payment of the nomination fee, will not be accepted by Hockey Queensland.
• Players seeking an exemption must fill out the nomination form, pay $40.00 and submit an exemption form (both forms available on HQ website).
• Associations are required to submit their team entry list to the HQ Championships as per HQ’s State Championships Schedule – Key Dates (approximately 1 week prior to Championships). After that date players will only be allowed to be added in extenuating circumstances with HQ approval. Any players added through that process shall be able to nominate for Queensland after the deadline has passed but must do so prior to 10.00am on the Wednesday prior to the Championships.
• All players who have nominated for selection must, at all times whilst playing, wear a visible white or maroon ribbon beside their shirt number.

Trials

Trials are not compulsory.

• 35s and 40s players must play for an Association team in the Championship to become eligible for selection
• 45s and 50s players who decide to trial must also play for an Association team in the Championship to become eligible for selection
• 55s, 60s, 65s and 70s players must either trial or play for an Association team in the Championship to become eligible for selection

Once nominations have been received, selectors will develop trialling groups and, by email, notify players of their trial time.

Trials on Thursday 9^th June will be held for Over 45, Over 50, Over 55, Over 60, Over 65, Over 70 players and Goal Keepers of all ages
