This form is to be completed by any players who do not wish to be considered for state squad/team selection from a tournament.
* By completing Notice of Ineligibility for State Selection Form, I acknowledge that my name/my child's name will NOT be included in the list of available/eligible players provided to the HQ selectors for the relevant HQ Tournament. Should your circumstances change and you/your child wish to be considered for selection, you must advise HQ Athlete Development Coordinator, Jordyn Wilson, via email jwilson@hockeyqld.com.au no later then 48 hours prior to the start date of the Tournament.
