This form is to be completed by any players seeking exemption from playing at a 2024 State Championships/Tournament or a state trials at which they would have been considered for State Team selection.
Applications for exemption must set out full reasons as to why the player is not available for the relevant selection event. Players in the National Open Squad/Team who are unable to compete are not required to complete an exemption form.
Exemption Requests can be submitted up until 2 days prior to respective championships or trials commencement. For requests submitted later than this, please notify Hockey Queensland.
Hockey Queensland will review all submissions prior to providing approved requests for exemption to relevant selection panel for consideration.
If seeking exemption on medical/injury grounds, you must provide a medical certificate to Hockey Queensland, prior to the commencement of the particular State Championships or Trials event.
