This form is used to submit Umpire Fitness Test results for FIH Umpires, Umpires on HA National Panels and QLD umpires who have been identified and listed on a QLD recommendation list for HA consideration.
Only HQ Approved Supervisors can witness/supervise Umpire Fitness Testing in QLD, and must submit results via this form.
HA have specific criteria for supervising, conducting and confirming the Umpire Fitness Tests that HQ have adopted and fully support. The Umpire Fitness Test guidelines and information can be found on the HQ Website via Umpiring tab
When submitting the umpire's Fitness Test result, the approved Witness agrees the umpire had reached the submitted level, witnessed the level achieved and was present for the entirety of the fitness test conducted.
