To be completed by the Association that is clearing the player
To be eligible to participate at a HQ Tournament, players must be registered with Hockey Queensland via their Association.
An eligible player may participate in a tournament in either of the following ways:
- For their Primary Association OR
- For an Association other than their Primary Association through the provision of a temporary ‘Player Clearance’ OR
- For The Hockey Queensland Championship for their Association of Origin (no clearance required)
Process for Providing a Player Clearance
The Association providing a clearance for a player must submit 'Player Clearance' prior to the deadline specified in HQ Tournament Regulations for each event.
When completing this form, the following information is required:
- Player Details
- Tournament Information
- Association the player is being given clearance to play for
