This form is to be completed if you wish to nominate for selection in a Queensland team to attend the 2021 Hockey Australia Mega Masters Championships in Newcastle, to be held from Thursday, 23^rd September to Saturday, 2^nd October, 2021.

Player Commitment

· Players selected in a Qld team, must be assembled in Newcastle by 6.00pm on Monday, 20^th September.

• The player levy is expected to be between $2500 and $2800 (plus air fare and uniforms). There is no change to the 2019 uniforms.

· The teams and shadow players will be named on HQ website on Wednesday 16^th June.

· Players and shadow players will be emailed their selection letter, including a confirmation link, shirt number and uniform ordering details and first invoice by Friday 18^th June.

· All team players must confirm their position in their team by Thursday 24^th June. Players who have not confirmed by 24^th June may be replaced.

· When confirming their position in a team, players must pay the first invoice of $1,400. A full breakdown of expenses will be listed in the second/final invoice.

· Any player seeking permission to be on a payment plan must contact HQ prior to Thursday 24^th June to establish the plan. All scheduled payments for players on a plan must be up to date on Wednesday 1^st September.

· Final invoices for all other players must be paid by Wednesday 1^st September.

· Accommodation has been booked for Saturday 2^nd October. Players may leave the contingent following the Closing Ceremony but refunds are not given for unused accommodation.

· Shadow players must confirm their selection by Thursday 24^th June but are not required to pay a deposit or order a uniform unless called into a team.

Nomination Information

• The $30.00 nomination fee is non-refundable
• Late nominations, and nominations which do not include payment of the nomination fee, will not be accepted by Hockey Queensland
• Players seeking an exemption must fill out the nomination form, pay $30.00 and submit an exemption form (both forms available on HQ website – Forms and Information)

· Trials on Thursday 10^th June will be held for Over 45, Over 50, Over 55, Over 60 & Over 65 players and Goal Keepers of all ages

• 8.45am Trial 1 65s/60s
• 10.00am Trial 2 65s/60s
• 11.30am Trial 3 55s
• 2.00pm Trial 4 50s
• 3.30pm Trial 5 45s
• 6.00pm Goal Keepers All ages

Trials are not compulsory.

• 35s and 40s players must play for an Association team in the Championship to become eligible for selection
• 45s and 50s players who decide to trial must also play for an Association team in the Championship to become eligible for selection
• 55s, 60s, and 65s players must either trial or play for an Association team in the Championship to become eligible for selection
• All players who have nominated for selection must, at all times whilst playing, wear a visible white or maroon ribbon beside their shirt number, which ever contrast better with shirt colour.

Supplementary Nomination Process

• This nomination category is for players who have been called into a team as a last minute emergency
• Supplementary Nominations must be completed online - the $30 nomination fee must be paid online at time of nomination
• Nominations can be submitted between 10.00am Monday, 7^th June and 10.00am Wednesday, 9^th June 2021. No nomination will be considered after this time
• A Supplementary Nomination will not be accepted from players who were included on Form 1 – Team Entry (submitted Monday, 10^th May 2021)
• A Supplementary Nomination will only be accepted from players who are listed on a team’s Form 7 – Pre-Tournament Team List (due by 10.00am Wednesday, 9^th June 2021)
