For almost a century, Australians from all walks of life have been involved in hockey, dedicating themselves to all aspects of the game, from the highest echelons right down to the grass roots.

The Hall of Fame will recognise those who have achieved excellence in the international playing arena and, by their achievements, significantly enhanced the image of the sport.

Hockey Australia will accept nominations for the inaugural year up to March 31, 2005. Inductees will be announced at the proposed 2006 Annual Hall of Fame and Awards Ceremony.

- State Associations
- Regional Associations
- Affiliated Hockey Clubs
- Past and present registered players
- Past and present Australian coaches, administrators and officials
- Life Members of Hockey Australia

Please complete the nomination form and send to Hockey Australia by March 31, 2005.
